    Association between a lifestyle score and all-cause mortality: A prospective analysis of the Chilean National Health Survey 2009–2010

    Objective: To investigate the association between a lifestyle score and all-cause mortality in the Chilean population. Design: Prospective study. Settings: The score was based on seven modifiable behaviours: salt intake, fruit and vegetable intake, alcohol consumption, sleep duration, smoking, physical activity and sedentary behaviours. 1-point was assigned for each healthy recommendation. Points were summed to create an unweighted score from 0 (less healthy) to 7 (healthiest). According to their score, participants were then classified into: less healthy (0–2 points), moderately healthy (3–4 points) and the healthiest (5–7 points). Associations between the categories of lifestyle score and all-cause mortality were investigated using Cox proportional hazard models adjusted for confounders. Nonlinear associations were also investigated. Participants: 2706 participants from the Chilean National Health Survey 2009–2010. Results: After a median follow-up of 10·9 years, 286 (10·6 %) participants died. In the maximally adjusted model, and compared with the healthiest participants, those less healthy had 2·55 (95 % CI 1·75, 3·71) times higher mortality risk due to any cause. Similar trends were identified for the moderately healthy group. Moreover, there was a significant trend towards increasing the mortality risk when increasing unhealthy behaviours (hazard ratio model 3: 1·61 (95 % CI 1·34, 1·94)). There was no evidence of nonlinearity between the lifestyle score and all-cause mortality. Conclusion: Individuals in the less healthy lifestyle category had higher mortality risk than the healthiest group. Therefore, public health strategies should be implemented to promote adherence to a healthy lifestyle across the Chilean population.

    A healthy eating score is inversely associated with depression in older adults: results from the Chilean National Health Survey 2016-2017

    Objective: To investigate the relationship of a healthy eating score with depression in Chilean older adults. Design: Cross-sectional study. Setting: Older adults from the Chilean National Health Survey 2016–2017. Associations were analysed using complex samples multivariable logistic regressions adjusted for age, sex, socio-demographic, lifestyles (physical activity, smoking, alcohol consumption and sleep duration), BMI and clinical conditions (hypertension, diabetes, hypercholesterolaemia and cardiovascular diseases). Participants: The number of participants was 2031 (≥ 60 years). The Composite International Diagnostic Interview-Short Form was applied to establish the diagnosis of major depressive episode. Six healthy eating habitswere considered to produce the healthy eating score (range: 0–12): consumption of seafood, whole grain, dairy, fruits, vegetables and legumes. Participants were categorised according to their final scores as healthy (≥ 9), average (5–8) and unhealthy (≤ 4). Results: Participants with a healthy score had a higher educational level, physical activity and regular sleep hours than participants with an average and unhealthiest healthy eating score. Participants classified in the healthiest healthy eating score had an inverse association with depression (OR: 0·28, (95% CI 0·10, 0·74)). Food items that contributed the most to this association were legumes (15·2%) and seafood (12·7 %). Conclusion: Older adults classified in the healthiest healthy eating score, characterised by a high consumption of legumes and seafood.

    Introducción: La lactancia materna (LM) es un factor protector contra la obesidad infantil; sin embargo, los mecanismos a través de los cuales ejerce este efecto aún no están claros. El objetivo fue describir los mecanismos asociados al efecto protector que ejerce la lactancia materna contra la obesidad infantil. Métodos: Se utilizaron los buscadores PUBMED, SCOPUS, Cochrane Library y Scielo para desarrollar una revisión descriptiva de la evidencia científica. Las palabras clave fueron: lactancia materna, obesidad, mecanismo y dieta. Se revisaron artículos en español e inglés, desde 1977 hasta el 2020. Resultados: El efecto protector de la LM contra la obesidad infantil está dado por una combinación de varios mecanismos, se destaca su composición nutricional y el aporte de sustancias bioactivas, algunas de ellas reguladoras de la ingesta energética. Los lactantes que reciben LM por más tiempo seleccionan alimentos más saludables en etapa preescolar, independiente de factores sociodemográficos. También han sido descritos efectos en la adiposidad, el control del peso corporal y la ingesta energética mediante regulación de la programación epigenética y de la microbiota intestinal. Conclusión: La LM es un proceso único, que interacciona de forma compleja con factores del crecimiento y desarrollo de los lactantes y preescolares. Su rol protector contra la obesidad ha sido asociado a diversos mecanismos. Sin embargo, se requiere de nuevas investigaciones para comprender los alcances que puede presentar la LM en la etapa pediátrica y su rol en la prevención de la obesidad.

    Introduction: The average life expectancy, as well as the prevalence of Type 2 diabetes (T2D), is increasing worldwide. Population-based studies have demonstrated that the duration of T2D has been associated with cognitive impairment. However, despite the high prevalence of T2D and cognitive impairment in Chile, the association between years with T2D and suspicion of cognitive impairment has not yet been investigated. The objective of this study was to investigate the association between duration of T2D and suspicion of cognitive impairment in Chilean older adults. Material and Methods: 1,040 older adults aged ≥60 years from the Chilean National Health Survey (2009-2010) were included. Suspicion of cognitive impairment was assessed by the abbreviated Mini-Mental State Examination (MMSE). The number of years with T2D was self-reported and categorised into four groups. Poisson Regression analysis was used to assess the association between altered MMSE and the number of years with DM2, adjusted by potential confounders including socio-demographic, lifestyle, adiposity and health-related factors. Results: When the analyses were adjusted for socio-demographic factors, people who had T2D for 15 to 24 and ≥25 years had 2.2-times (95%CI: 1.07; 3.33) and 5.8-times (95%CI: 3.81; 11.0) higher relative risk (RR) of cognitive impairment, compared to those without T2D. When the analyses were additionally adjusted for lifestyle and health-related covariates, the RR for cognitive impairment was 1.76-times (95%CI: 1.02; 2.50) and 4.54-times (95%CI: 2.70; 6.38) higher for those who had T2D for 14-24 years and ≥25 years, respectively. Conclusions: Number of years with T2D was associated with suspicion of cognitive impairment. A longer duration of T2D was associated with a higher likelihood of cognitive impairment in the Chilean older population, independently of confounder factors included in the study.

    Suplementos ergogénicos: la evidencia más allá de una moda

    A pesar de que son muchos los suplementos deportivos que se etiquetan como ergogénicos, pocos son los que realmente demuestran su efectividad. Si bien estos suplementos están destinados principalmente para deportistas de élite, su uso se ha masificado en la población general, esto debido a la falta de regulación en uso y venta, así como por los potenciales beneficios que podrían otorgar. Frente a estas limitantes ¿cuál es la efectividad de estos productos más allá de la moda? En esta comunicación breve se revisa la evidencia respecto al efecto y la efectividad de 11 suplementos y plantas utilizados como ayuda ergogénica. A pesar de los múltiples beneficios en los suplementos ergogénicos identificados, es necesario tener precaución al momento de generalizar los resultados obtenidos por algunos ergogénicos, ya que su efecto podría ser secundario y/o estar condicionado por otros factores. Es necesario mayores estudios clínicos que permitan asociar una relación causa-efecto en el rendimiento deportivo y la población general.

    Cancer is a chronic non-communicable disease associated with a high mortality burden. The prevalence of cancer is increasing rapidly worldwide. However, this scenario will be worse in low and middle-income countries such as Chile, where 70% of cancer deaths occur. The aim of this review was to assess the epidemiological scenario of cancer and its projection for the Chilean population. In Chile, 53,365 new cases of cancer were diagnosed in 2018, led by prostate, colorectal, breast, stomach, lung and gallbladder cancer. From 1986 to 2016, cancer increased by 109%. When we reviewed mortality by sex, stomach and prostate cancer were responsible for more than 30% of cancer deaths among men. However, for women the first three places were occupied by breast, colorectal and lung cancer, as in the rest of the world. Considering that 40% of cancers are related to unhealthy lifestyles, working on the prevention of modifiable risk factors represents an opportunity for the creation of public health policies that allow changes at the environmental and individual level.

    La mayoría de las enfermedades cardiovasculares se atribuyen a factores de riesgo que podrían ser modificados con cambios de los estilos de vida

    Las enfermedades cardiovasculares (ECVs) son la principal causa de muerte en Chile, representando 27,1% del total de defunciones en el año 2016, con una tasa de mortalidad por accidente cerebrovascular e infarto de miocardio de 46,4 y 44,8 por 100.000 habitantes, respectivamente. Por otra parte, más de 70% de los casos de ECVs son atribuibles a factores de riesgo modificables reportó el estudio prospectivo PURE (Prospective Urban Rural Epidemiology). La reciente pesquisa liderada por Yusef et al. midió la asociación entre enfermedad cardiovascular y 14 factores de riesgo modificables en 155.722 adultos pertenecientes a 21 países, de los cuales 3.573 eran chilenos. Entre los factores de riesgo incluidos en este estudio están el tabaquismo, consumo de alcohol, calidad de la dieta, actividad física, excreción urinaria de sodio, hipertensión, diabetes, colesterol no-HDL, obesidad abdominal, nivel educacional, depresión, fuerza de prensión y la contaminación del aire doméstico (uso de queroseno o combustible sólido para cocinar y/o calefacción).

    Obesidad en lactantes: efecto protector de la lactancia materna versus fórmulas lácteas

    El objetivo del trabajo fue analizar el posible efecto protector de la lactancia materna respecto a la obesidad infantil versus fórmulas lácteas. Se consultaron las bases de datos PubMed, Web of Science y Scielo, considerándose los artículos más actualizados sobre el tema. La evidencia analizada señala que los menores alimentados con lactancia materna exclusiva reportan bajos niveles de sobrepeso y obesidad, a diferencia de los alimentados con fórmulas lácteas infantiles. Se concluye que la lactancia materna exclusiva, hasta el sexto mes de vida del lactante, y complementada con alimentación solida (no láctea) hasta los dos o más años, es recomendada y podría proteger contra la obesidad infantil y sus comorbilidades asociadas en la vida adulta.

    Nivel de actividad física y sedentarismo en personas con diagnóstico de cáncer en Chile

    Background: One of the side effects of cancer treatment is a low level of physical activity (PA) due to fatigue and pain. Aim: To quantify PA levels in Chilean people with cancer. Material and Methods: Analysis of the National Health Survey 2016-2017 including 6,233 participants, comparing those with and without a self-report of cancer was conducted. Levels of PA and sitting time were assessed using the Global Physical Activity Questionnaire. Results: 3% of participants [confidence intervals (CI): 2.6; 3.9] reported having cancer and 34% [95% CI: 22.7; 46.7] of these reported being physically inactive, compared to 25% [95% CI: 22.5; 27,3] of those without cancer. Participants with cancer performed 56 and 77 minutes/day lower vigorous and total PA than participants without cancer. No differences between groups were observed for commuting PA, moderate PA and sedentary time. Lower level of PA was independent of the years elapsed since the diagnosis of cancer. Conclusions: People with cancer are less physically active than their counterparts without the disease.
